Horizon 2020 research funding was granted to SRI for the 2015-2019 time period, as a member of the SELFIE consortium, led by Erasmus Universitair Medisch Centrum Rotterdam.
The project aims to improve patient-centred care for patients with multi-morbidity by proposing evidence-based, economically sustainable integrated chronic care (ICC) models that stimulate cooperation across health and social care sectors and are supported by appropriate financing/payment schemes.
